What are your billing requirements?
Any co-payments need to be paid at the time of each patient visit. Patients are responsible to pay any amounts, such as deductibles, co-payments and co-insurance that their insurance company designates is their responsibility.

Please beware that in the rare circumstances, active in-network members become inactive or out-of-network due to the insurance company rejecting claims for reasons including premiums not paid by members to the insurance company on time, the length of time it takes for updated paperwork to be accepted, and changes in insurance contracts.
It is recommended that patients stop by the reception desk to find out the latest updates and verify their insurance before each visit.

How should I prepare for my appointment?
You need to bring two things to your appointment: (1) your ID (2) your insurance card. It is always helpful to prepare any papers/information you have relating to your health or insurance, and bring them with you to the doctor’s visit. The physician will need to know about your medical history in order to properly assess your issue and provide a treatment plan. It is also a good idea to write down any questions or concerns you want to share with the doctor during your visit.
